The sponsored trip is a month-long trek across western South America, exploring the beauty, fun, and perils of Peru while in search of the historically famous El Dorado. Join your school staff as you encounter stunning views, dangerous creatures, and adventures at every corner. FIELD TRIP PERMISSION SLIP

I hereby grant permission for my son/daughter/ward _____________________ to participate in a summer field trip across seas to South America from July 1 2083 - July 31 2083. I understand this field trip is optional and attendance by my child is not required, nor will it affect my child's position or grade at Hogwarts School of Witchcraft and Wizardry if my child elects not to participate.

Transportation for this field trip will be provided by portkey.

The planned field trip is tentatively scheduled to leave the Leaky Cauldron at 10:00 AM, JULY 1 2083 and return to the Leaky Cauldron at approximately 5:00 PM, JULY 31 2083. A letter will be sent to parents/guardians if a decision has been made to arrive or dismiss students to parents/guardians at an alternative location.

I understand that all students participating in this trip will be responsible in conduct to the teachers and adult sponsors at all time. I further understand that students are required to go and return from this event on the transportation provided, unless prior arrangements have been made.

Authorization to treat a minor: In the event that I cannot be reached in an emergency, I hereby give permission to the physician selected by the school staff to secure proper treatment for my child.

I understand that the Hogwarts' Board of Governors provide up to 50 percent financial compensation for this trip with the provision that all persons making a field trip or excursion of their own choosing shall be deemed to have waived all claims against the Board or the School for injury, accident, illness or death occurring during or by reason of the field trip or excursion.

OOC: So! A field trip! We'll be gathering in the Leaky Cauldron starting Sunday April 13, and will begin our adventure in a thread created in the Vanishing Cabinet RPG forum on April 17 [tentatively ending April 30]. Sign-Ups will be open until the thread in the VC is created, so make sure you post here before then if you'd like to join in, or risk missing out! For now, if your character will be a 5th - 7th year during the Term 37 school year, feel free to sign up! Your character must attend Hogwarts for the 2083-2084 school year to participate.
